Nonatopic atopic dermatitis versus atopic dermatitis in children

The com­plex in­ves­ti­ga­tions we­re per­for­med in 134 chil­dren with ato­pic der­ma­ti­tis (AD). Res­pi­ra­to­ry al­ler­gy was ex­clu­ded. The con­trol group com­pri­sed 17 he­alt­hy and suf­fe­ring from re­cur­rent res­pi­ra­to­ry  di­se­a­ses chil­dren wit­hout any signs of al­ler­gy. On the ba­sis of cli­ni­cal, al­ler­go­lo­gi­cal, im­mu­no­lo­gi­cal and me­ta­bo­li­cal fin­dings three ty­pes of chil­dren AD we­re de­ter­mi­ned: I ty­pe – ato­pic der­ma­ti­tis (IgE me­dia­ted), II ty­pe – no­na­to­pic (non-IgE me­dia­ted) ato­pic der­ma­ti­tis or me­ta­bo­li­cal, III ty­pe - com­bi­ned with as­so­cia­ted im­mu­no­lo­gi­cal (IgE me­dia­ted) and me­ta­bo­li­cal chan­ges. This ty­pe is the most se­ve­re and the most re­sis­tant to tre­at­ment. De­la­y­ed ty­pe hy­per­sen­si­ti­vi­ty (T cell-me­dia­ted) was not in­ves­ti­ga­ted. The pe­cu­lia­ri­ties of cli­ni­cal ma­ni­fes­ta­tions and cri­te­ria for diag­no­sis of tho­se ty­pes we­re re­ve­a­led. Not on­ly IgE me­dia­ted al­ler­gy, but so­me ot­her con­di­tions can mi­mic al­ler­gies in chil­dren with AD. The die­ta­ry me­a­su­res in chil­dren with AD con­tain two prin­cip­les: the first - eli­mi­na­tion of cau­sa­ti­ve fo­od al­ler­gens; the se­cond - li­mi­ta­tion of fo­ods, rich in such ami­no­a­cids, tho­se chan­ges we­re re­ve­a­led ac­cor­ding to the pre­va­lent ty­pe of AD. Chil­dren with AD al­so ne­ed en­vi­ron­men­tal me­a­su­res of in­do­or en­vi­ron­ment con­trols, be­cau­se of IgE me­dia­ted sen­si­ti­za­tion to in­do­or al­ler­gens, not on­ly to fo­ods, was re­ve­a­led.
