BETWEEN GUIDELINES AND DAILY PRACTICE: TWO CASES OF PAEDIATRIC PNEUMONIA Anne Thomson Department of Paediatrics of John Radcliffe Hospital, Oxford, UK
The article presents two different clinical cases of childhood pneumonia diagnosed and managed according to recent BTS and project of ERS Guidelines for the Management of Community Acquired Pneumonia in Children. Comments are made to both clinical cases with particular interest in complicated case of pneumonia of 4 years girl, that developed on the base of asymptomatic congenital cystic adenomatous lung malformation. Congenital lung malformations should be suspected in children under 6 years of age with persistent symptoms of pneumonia in whom adequate treatment according to approved guidelines for the management of pneumonia was administered. During diagnosis and treatment of pneumonia in childhood proper medical history and age of a child should be taken into consideration when primary care doctor or hospital consultant makes his choice in laboratory, instrumental diagnostic tools, and empiric first line antimicrobial medicine.
