PLASTIC BRONCHITIS: NEW INSIGHTS AND A CLASSIFICATION SCHEME

Plastic bronchitis is a rare disease in which there is the formation of large gelatinous or rigid branching airway casts. Authors propose a new classification scheme first based on the associated disease and second on cast histology when comorbidity is unclear. They also provide evidence that this proposed classification can be prognostic for outcome and can be used to guide treatment. With plastic bronchitis associated diseases are structural (particularly cianotic) congenital heart diseases, lymphatic disorders, asthma and atopic conditions and sicle cell acute chest syndrome. Authors describe structure of casts acording associated disease – inflammatory, noninflamatory, mucinous, chylous, fibrinous casts, and possible outcome.
